Accepted Student Forms

2017 Student Forms will be available after January 16.

Once you receive your acceptance notification (via email) you must download and fill out the Required Form to complete your registration process. Please mail this component along with your final balance payment due May 24. Your registration will not be complete without receipt of this final package.

Required Form

 Room Key Deposit

An $80 key deposit payment, due with your final payment on May 24, 2017, is required to complete enrollment.  This payment may be made only by check and must be postdated July 19, 2017.  Your check will be held and will only be deposited if your key is not returned.

  Health Insurance Form

A copy of both side of your health insurance form must be faxed or mailed by May 24 to complete your enrollment package.
